You must complete a registration form, providing your current address, phone number and date of birth. A driver's license is the preferred form of photo identification, however, other forms of photo identification may also be used. If your license does not show your current address, please provide proof of residence from the Post Office (mail showing your current address is acceptable).
Children 12 and under must apply with a parent or guardian and that adult’s signature is required on their registration form. Patrons must be at least 5 years old.
